To start the process of a legal adoption, you must file a Petition for Adoption in court. There are no official self-help forms for adoption in Wyoming. You may need assistance from a lawyer to help write the Petition.
Wyoming Vital Records
Wyoming Vital Record Services (also called Vital Statistics) is a government office that may have some of the forms and information you'll need for an adoption. It's important to know that the Report of Adoption form and the Adoption Application/Request for Birth Certificate Following Adoption form are only part of the adoption process. These forms alone do not start the adoption process.
