Family Law Forms

This information is provided as a service by the Supreme Court of the State of Wyoming. These packets are designed to assist you in representing yourself in court matters. The packets will be more helpful to people who agree on the terms of their divorce, child custody or child support case.

Criminal Notice of Appeal Forms

Use the “District Court Notice of Appeal Forms” to appeal a criminal judgment and sentence that was decided in a Wyoming district court.

Use the “Circuit Court Notice of Appeal Forms” to appeal a criminal judgment and sentence that was decided in a Wyoming circuit court.

The “Affidavit of Indigency and Request for Waiver of Filing Fees” can be used for either type of appeal.
